- 4 Passport size and 2 Stamp Size Photographs of student
- Eligibility Certificate from Maharashtra University of Health Sciences(MUHS), Nashik
- National Eligibility cum Entrance Test(NEET)
- Physical Fitness Certificate with marks of identification
- Scheduled Caste/Scheduled Tribe students should produce Caste & Income Certificate
- S.S.C. (10th Std.) Marks Card
- H.S.C. (12th Std.) Marks Card
- Migration Certificate by Board
- Blood Group Report
- Transfer Certificate
- Date of Birth Certificate
- Passport & Visa copies in case of foreign students
